The Challenge: Substance Use Disorders

Substance use disorders (SUDs/Addiction) account for 5.4% of the world's annual disease burden. While tobacco and alcohol have been the major legal substances of abuse in India, there has been a rapid and substantial increase in the use of marijuana, newer drugs of abuse (methamphetamine, LSD) and pharmaceuticals.

Adolescents are experimenting with drugs much earlier, placing them at a higher risk for addiction and related harm. Substance use has a substantial impact not only on the user's physical and mental health, but also has devastating consequences for the family and society at large.

The Treatment Gap: The growing problem of addictions on one hand and lack of trained service providers on the other has led to a huge "treatment gap" up to 75%. There is an urgent need to bridge this treatment gap in this specialized area of public health importance.

About NIMHANS Digital Academy ECHO: Democratizing Knowledge

NIMHANS Digital Academy ECHO uses HUB and SPOKE knowledge-sharing networks, led by expert teams of NIMHANS (an academic center). They use smartphone app multipoint videoconferencing to conduct virtual sessions with community health providers.

Focus of Weekly Sessions

  • Case-based learning to understand the complexities
  • Following best practices like algorithms, guidelines along with clinical wisdom for knowledge diffusion
  • Using technology to leverage scarce resources
  • Web-based monitoring for outcomes

These live virtual sessions are accessible through smartphones and are linked to a mobile e-learning certification/accreditation through which learners can access learning materials as well as relevant questions anytime and anywhere.

Proven Impact

In a recent study, 26 primary health care providers could screen 2,695 individuals and identify 832 (30.8%) as Alcohol use disorders. The PCPs reported retaining 49.1% of the cases for at least one follow-up and needed to refer only 80 (3%) cases to specialists in higher centers for further management.

In another study, primary care physicians could provide care to 23,000 patients and rated an average of 4.5/5 on learning through NIMHANS ECHO tele-mentoring model.

State Drug Dependence Treatment Center (SDDTC), Institute of Mental Health is an apex center for the treatment and training in substance use disorder management in the state of Haryana.

Services & Facilities:

  • Both inpatient and outpatient services for SUDs
  • Dedicated manpower with consultants in Psychiatry, psychology and social work
  • 18 bedded inpatient facilities with recreational, occupational and integrated yoga services
  • Training for postgraduate psychiatry, psychiatry social work and nursing students

Partnering with ECHO India and NIMHANS Digital Academy, SDDTC initiated online training for the management of substance use disorders for professionals from different backgrounds in the state of Haryana.

Course Objectives

  • Differentiate various factors leading to Addiction/abuse by integrating hereditary, neurobiological and psycho-social factors for a holistic approach
  • Incorporate screening and assessment for Alcohol use disorder in practice
  • Increase ability to decide safe and appropriate pharmacological agents and psychosocial intervention as per evidence-based practice for AUD
  • Decide when to refer the patient to an expert

Core Competencies

What a CCBOAM Certified Champion CAN Do:

  • Effectively assess patients for Alcohol Use Disorders
  • Detoxify a person with Alcohol Use Disorders (for doctors)
  • Provide anti-craving medications for AUD (for doctors)
  • Use brief intervention and components of Motivational Interviewing
  • Implement relapse prevention strategies

What a CCBOAM Certified Champion CANNOT Do:

  • Start specific treatment for Opioid, cocaine, Stimulant use disorders
  • Manage patients with addiction and severe mental disorders like psychosis
  • Call themselves an Addiction Psychiatrist or psychiatrist

Accreditation Criteria

To receive the Certificate of Accreditation, participants must fulfill ALL of the following criteria:

  1. Attendance: More than 75% attendance (11/14 live sessions) with >60 minutes video presence for each session
  2. Case Presentation: One case presentation during weekly live tele-ECHO session
  3. Assignments: Completion of three e-Assignments with score >75% for each (second attempt allowed for those who take first attempt)
  4. Patient Reports: Monthly reports of patients seen/evaluated during the course and six months after
  5. Pre-test and Post-test: Completion of both assessments (no scoring involved)

Important Note: Each assignment has TWO attempts to clear (>75%). Each assignment must be completed within 48 hours of being assigned. There is NO participation certificate.
